If you live in our practice area and would like to register with us, please complete one of our registration forms and a brief questionnaire, available from our reception. You can also download the forms by following the links below. On returning the form, please inform the receptionist if you would prefer to see a particular doctor. However, you will be registering with the practice rather than an individual GP and if you need to make an appointment you can also express your preference then.
Please refer to the consultation times at under Practice Information for the doctors surgery hours.
New patients should make an appointment to see the Practice Nurse within one month of registering for a general health check
Please note you must provide the name of your previous doctor. If you do not know, or cannot remember, please contact your previous surgery for the necessary information. We need this information to be able to register you with our practice.
You will need to complete an NHS (GMS1) Registration Form for each member of your household when you register in person. This is only available from the Surgery.
As it takes a while for your medical records to transfer to our practice we also ask you to also fill in our 'personal history' form for each person being registered.
